And if Corrective Action get chosen, it flows to analyzer for analysis and analyzer will do it through any or more of following tools Root-Cause analysis, 4W-1H analysis, Benefit Analysis. Then it flows back to manager for approval. After it gets approval, task or tasks is/are created with its performer, reviewer and probable completion date. After each task/tasks got reviewed, Corrective Action against source issue is flows to approver for approval.

Again as Issue there could be moved into a hierarchy of Preventive Action or Improvement Action.
